Positions Available:

HOSPICE MEDICAL DIRECTOR

Henry County Health Department Home Health/Hospice program is seeking a contract Medical Director for our hospice program.

Qualifications/Licensure Requirements:

  • A physician (M.D. or D.O.) licensed to practice medicine in the state of Ohio
  • Expertise in the medical care of terminally ill individuals
  • An ability to provide phone and/or in-person consultation with agency staff and hospice patients (averaging ~five to ten hours monthly)

Responsibilities include:

  • Providing clinical physician services to Hospice patients through assessment and/or prescribing medications or treatments
  • Serving as collaborating physician for Hospice patients’ primary care physician (or serving as the patient’s primary physician if no primary physician is available or willing to follow)
  • Attending and acting as the Physician Representative for the interdisciplinary group meeting (IDG) (one hour every two weeks)
  •  Reviewing patient criteria for hospice eligibility and completing and/or co-signing initial hospice certifications
  • Performing face-to-face visits to clients for re-certification for subsequent election periods that the patient is terminally ill (on average 2-4/month)
  • Assuring that the patient receives appropriate interventions to control uncomfortable symptoms
  •  Providing 24/7 consultation services for hospice emergencies

QUALITY IMPROVEMENT COORDINATOR

Henry County Health Department is seeking to fill one, part-time (21 hours) Quality Improvement Coordinator position.

Qualifications/Licensure Requirements:

Responsibilities include:

  • Managing agency’s performance management and quality improvement system; participating in organizational strategic planning and providing leadership for Performance Management and Quality Improvement policy development.
  • Facilitating agency Quality Council, developing Quality Improvement Plan and ensuring objectives of plan are met.
  • Utilizing Performance Management/Quality Improvement tools, methods, and processes to improve operational and program efficiencies and effectiveness.
  • Researching grants and other funding opportunities.

The position will be part- time.  This position is in Pay Grade 8; pay range $21.25-$29.75/hour.

HELP ME GROW HOME VISITOR

Henry County Health Department’s Help Me Grow Program is seeking to fill one, part-time Help Me Grow Home Visitor position to provide services in Fulton and Henry Counties. Prefer candidates with previous home-based experience.

Qualifications/Licensure Requirements:

Responsibilities include:

  • Traveling and conducting home visits to provide support and education to assist families with their child’s development, health and safety, basic home management, and appropriate discipline practices
  • Developing Family Plans with family, assisting with referrals and making appointments for the child.
  • Preparing required records and reports to document home visits and other contacts
  • Completing required Help Me Grow training
  • Completing required activities/reports/paperwork within set Help Me Grow timelines

The position(s) will be part- time, contingent upon SFY2017 funding. This position cannot be combined with other duties or position(s) currently held. This position is in Pay Grade 6; pay range $17.57-$25.47/hour.

PUBLIC HEALTH 2 NURSE/HOME HEALTH & HOSPICE

Henry County Health Department Home Health/Hospice program is seeking a full-time registered nurse for the position of Public Health Nurse 2.

 

Qualifications:

Graduate of an accredited school of nursing plus two (2) years of prior experience in medical/ surgical nursing; experience in home health/hospice preferred; or equivalent combination of education, training, and experience; bilingual (Spanish) skills preferred.

Must be a Registered Nurse; have or obtain CPR certification (American Heart Association); maintain State of Ohio driver’s license; maintain insurability according to Health District policy.

 

Responsibilities:

  • The delivery of services to home health/ hospice program participants; interviews and assess patients to determine needs for services, develop and administer medical plans based on patient needs, works with all disciplines  and the patient to establish a comprehensive plan of care.
  • Performs comprehensive assessments, provides skilled interventions aimed at achieving outcomes and goals with in specified time periods; modifies, evaluates and updates plan of care to reflect patient needs and outcomes; consistency demonstrates competency with technical nursing skills.
  • Conducts home visits to provide routine and specialized direct nursing care.
  • Prepares required records and accurately maintains records for submission in a timely manner.
  • Provides for the safety of the patient.
  • Provides for the education appropriate to the patient’s/family needs
  • Provides guidance to Home Health Aides.
  • Demonstrates regular and predictable attendance; must be able to work assigned evenings, weekends, and holidays.
  • Must maintain State of Ohio driver’s license, must maintain insurability according to policy

 

Position will be full-time, working 35 hours/week. Position is non-exempt from overtime.

 

Positions are in Pay Grade 7; pay range $19.32 – $27.04/hour. Current average pay range is $22.30-$24.00.
