Positions Available:

LICENSED PRACTICAL NURSE (LPN) / HOME HEALTH & HOSPICE

The Home Health/Hospice program is seeking a new full-time Licensed Practical Nurse (LPN) position.

Qualifications:
Two years of experience as a Licensed Practical Nurse (LPN) or equivalent combination of education, training, and experience; bilingual (Spanish) skills preferred.
Must be a Practical Nurse licensed in the state of Ohio; must have IV Therapy Certification, cardiopulmonary resuscitation (AHA), and first aid certifications; must maintain State of Ohio driver’s license and insurability according to Health District policy.

Responsibilities:

This position administers nursing care, provides skilled interventions, and performs comprehensive assessments in patient homes as assigned. Coordinates referral process; meets with walk-in clients and families for education on services available. Completes proper documentation via electronic medical record and conducts chart audits.

This position must demonstrate regular and predictable attendance and be able to work assigned evenings, weekends, and holidays.

Hours and Pay Range:

Position will be full- time (35 hours/week) and is non-exempt from overtime. Benefits include paid vacations, holidays, sick leave and retirement through Public Employee Retirement System

Position is in Pay Grade 6; pay range $17.57 – $25.47/hour.

PUBLIC HEALTH 2 NURSE / HOME HEALTH & HOSPICE

Henry County Health Department’s Home Health/Hospice Program is seeking to fill a part-time Public Health Nurse 2 position.

Qualifications:

Graduate of an accredited school of nursing plus two (2) years of prior experience in medical/ surgical nursing; or equivalent combination of education, training, and experience; experience in home health/hospice and Oasis documentation preferred; bilingual (Spanish) skills preferred.

Must be a Registered Nurse; have or obtain CPR certification (American Heart Association); maintain State of Ohio driver’s license; maintain insurability according to Health District policy.

Responsibilities:

  • The delivery of services to home health/ hospice program participants; interviews and assess patients to determine needs for services, develop and administer medical plans based on patient needs, works with all disciplines and the patient to establish a comprehensive plan of care.
  • Preforms comprehensive assessments, provides skilled interventions aimed at achieving outcomes and goals with in specified time periods; modifies, evaluates and updates plan of care to reflect patient needs and outcomes; consistency demonstrates competency with technical nursing skills.
  • Conducts home visits to provide routine and specialized direct nursing care.
  • Prepares required records and accurately maintains records for submission in a timely manner.
  • Provides for the safety of the patient.
  • Provides for the education appropriate to the patient’s/family needs
  • Provides guidance to Home Health Aides.
  • Demonstrates regular and predictable attendance; must be able to work assigned evenings, weekends, and holidays.
  • Must maintain State of Ohio driver’s license, must maintain insurability according to policy

This is a part-time position and is in Pay Grade 7; pay range $19.32 – $27.04/hour
