Children with Medical Handicaps (CMH)

About CMH

CMH, or the Children with Medical Handicaps program (formerly known as BCMH), is an Ohio Department of Health program available to eligible families in every Ohio county. CMH links families of children with special health care needs to a network of quality providers and helps families obtain payments for the services their children need.

Our public health nurses work with families on behalf of children with special health care needs who reside in Henry County.
Public Health Nurses work with families and CMH to:

    • Help identify eligible children with special health care needs
    • Help families fill out required forms for CMH and explain paperwork
    • Help families locate CMH-approved providers and use services they need
    • Make referrals to doctors, pharmacies, durable medical equipment providers, and other services

Eligibility and Enrollment

  • The Ohio Department of Health determines the medical and financial eligibility criteria for children under the age of 21 years, have special health care needs, and are residents of the State of Ohio.
  •  To enroll in CMH, a Medical Application Form must be submitted by a CMH-approved physician. Our public health nurses can provide a referral and start the enrollment process, or you may contact 800-755-4769.
