This Clinic receives
funding from the
Ohio Department of Health Womens Health Services Grant and Title X
Family Planning Grant.

Henry County Family Planning Clinics
are held weekly, by appointment at the Henry County Health
Department. We also provide a satellite clinic in Defiance
County two to three times monthly, by appointment. Call (419)
599-5545 or 1 (877) 591-3050 to make an appointment. All
services are provided by registered nurses, nurse practitioners and
physicians.
Services Include:

Physical examinations including pelvic exam, pap
smear, and clinical breast exams
·
STD testing and treatment for males and females
·
Pregnancy testing, counseling, and referral
·
Level 1 infertility testing
·
IUD insertion and removal
·
HIV testing
·
Laboratory testing as ordered by clinician
·
Abstinence Education
·
Birth control including oral contraceptives,
depo provera injection, diaphragms, natural
family planning instruction, condoms, IUDs, vaginal ring and emergency
contraception.
Costs of service are
based on an income based sliding scale fee.
Application
Medicaid and some insurances are accepted.
No one is denied
service due to an inability to pay. |
