A community health assessment (sometimes called a CHA), refers to a state, tribal, local, or territorial health assessment that identifies key health needs and issues through systematic, comprehensive data collection and analysis. CHAs also help measure how well a public health system is fulfilling its function.
What Makes Us Healthy?
Health is more than what happens at the doctor’s office. A wide range of factors influence how long and how well we live from education and income to what we eat and how we move to the quality of our housing and the safety of our neighborhoods.
For the 2023 Community Health Assessment process the Henry County Health Partners used the National Association of County and City Health Officials’ Mobilizing for Action through Planning and Partnerships (MAPP). MAPP is a community-driven strategic planning process for improving community health by looking at health through a lens of health equity and community engagement. The three assessments are:
