Birth Certificate Requests

Henry County Health Department can provide certified birth records (certificates) for anyone born in the state of Ohio after December 20, 1908.

Request a certificate in person

  • Monday through Friday 8:30am-4:15pm
  • We accept cash, check, money order, or credit/debit card (additional fee applied) for same day service.
  • $25.00 for each certified copy requested. 

Request a certificate via mail

  • Send completed Vital Statistics Records Request Form with required $25.00 fee (check or money order payable to Henry County Health Department) to the address on the contact us page.
  • Orders are processed within 2 business days.  Please allow 7 – 10 business days for the return of your birth certificate.
  • $25.00 for each certified copy requested. 

Request a certificate via phone

  • You can now order your birth certificate by telephone using a major credit/debit card (MasterCard, Visa, and Discover). 
  • Please call our office at 419-599-5545 option 5. Please note we will mail purchased birth certificate out USPS the same day or within one business day. Please allow 7 – 10 days to receive certified birth certificate.
  • $25.00 for each certified copy requested. Additional fees for credit/debit card usage.

Additional Information:

Vital records (records of births, deaths, and fetal deaths) are public records in Ohio. This means that anyone who can submit the basic facts of a record may request a copy.

Birth records will be issued as certified abstracts unless you indicate that you are requesting the certified copy for the specific purposes of obtaining dual citizenship, international marriage or legal proceedings or genealogy.

Notice to All Vital Statistics Customers: Pursuant to Ohio Revised Code 3705.29, it is unlawful to purposely obtain, possess, use, sell, furnish, or attempt to obtain, possess, use, sell, or furnish to another for the purpose of deception any certificate, record, or certified copy of it that relates to the birth of another person, whether living or dead.
