About the Program:
The Henry County Cares program is a free program that pairs you with a trained professional from your community called a Community Health Worker (CHW) to help improve your overall health. A CHW will meet with you to identify your needs; then will connect you to the social service programs and medical providers to help you.
What You Can Expect:
After you complete the referral form, we must check if you qualify to enroll in the program. If you qualify, a staff person will contact you within 2 business days to answer your questions about the program and assign you to a CHW.
