About the Program:

Henry County Cares links you to the services and resources you need. Our community health workers (CHWs) help you get the care you need, connect you to resources, teach you how to take care of any health issues, and support you every step of the way.

Our CHW staff can meet you in your home, at the department, and attend appointments with a doctor or community agency

Our bilingual staff can provide our services in Spanish for Spanish speaking clients
