About the Program:
What You Can Expect:
After you complete the referral form, we must check if you qualify to enroll in the program. If you qualify, a staff person will contact you within 2 business days to answer your questions about the program and assign you to a CHW.
If you do not qualify to enroll, we will contact you to discuss your needs and provide information and referral to help you.
If you are using this online referral source, please allow 2 business days for us to get back with you. For immediate service you may call 419-599-5545 option 4, Monday – Friday between 8:30am-4:30pm. HCHD is closed on all major holidays.
