Community Health Improvement Plan
The community health improvement plan (or CHIP) is a long-term, systematic effort to address public health problems based on the results of community health assessment activities and the community health improvement process. A plan is typically updated every three to five years.

From the Community Health Assessments, our partners identified three priority areas where we can make the most impact on the health of Henry County:
Financial Stability
Financial stability is an integral part of complete well being. Financial stability is a closely related determinant of health for both physical and mental health.
Physical Health
Physical health and it’s contributing factors are key to reducing the mortality and the total burden of disease and illness for residents of Henry County.
Mental Health
Mental health is crucial because it affects emotional well-being, relationships, overall physical health, and how we cope with stress and navigate daily life.

2023-2025 Community Health Improvement Plan Final Report
From 2023 through 2025, Henry County Health Partners implemented a coordinated Community Health Improvement Plan (CHIP) focused on four priority areas: Access to Care, Housing, Mental Health, and Substance Misuse. This report highlights the work that was completed over the three-year period by agencies that are a part of the four priority work groups.
