Community Health Improvement Plan
The community health improvement plan (or CHIP) is a long-term, systematic effort to address public health problems based on the results of community health assessment activities and the community health improvement process. A plan is typically updated every three to five years.

From the Community Health Assessments, we identified four priority areas where we can make the most impact on the health of Henry County:
Access to Care
Expanding access to health insurance to limit out of pocket expenses for Henry County Residents that may cause them to forgo treatment.
Housing
Tackling the housing shortage in Henry County can help provide the very foundation of what people need to start making better choices for health.
Mental Health
Mental health is crucial because it affects emotional well-being, relationships, overall physical health, and how we cope with stress and navigate daily life.
Substance Misuse
Substance Use Disorder is a vicious cycle harming mental, physical, and emotional health of our residents. Let’s stop this together as a community.

HCHD hosts a meeting with partners, organizations, and interested community members for each of the priority areas every quarter. Feel free to come and see what we are doing, and share how you think we could solve the issue as a community.
